As the season begins lets not forget that these are children playing a game.  A game that we love so much, not only to watch but to engage with play and cheer for.  As we all know this is an outdoor sport and we as parents and board members know things happen on and off the field. Below are a few forms for those such occasions. As many of you know it is important to document everything and these forms will help you with that.

Incident Form- Should be filled out in the event an accident occurs on the field, players may get hit with balls or get hurt sliding.
